Reporting Documents
JSFC Incident Report - This document can be used by an official, coach, administrator or parent to report on an incident that has occurred at a regularly scheduled JSFC Football Game. It is a word document and must be completed and emailed to the Competition Committee and JSFC President by Wednesday following the indicent to be considered for investigation.
JSFC Official Disqualification Ejection Incident Report - This document is used by an official any time a player, coach or fan is ejected from a game. It is to be completed in its entirety and emailed to the competition committee by Wednesday following the date of the ejection.
JSFC Officiating Crew Game and Program Evaluation Form - This document is for use by each games officiating crew. It is not required but can be used at the discretion of each crew weekly to give general comments on the location of the game the teams and the fans. Completed forms must be emailed to the competition committee by Wednesday following each game or they will not be considered.
JSFC Officiating Crew Review Form - This form is to be completed after every game by both the home and away team to evaluate the officials performance in the game. The fully Completed review form must be e-mailed to JSFC Competition Committee by Wednesday following each game or the review will not be counted and each member on the crew will be considered to have attained a perfect score.
Lopsided Score Rule Explanation - This form is to be completed by the head coach of any team that wins a game by 35 points or more and must be emailed to the Competition Committee by Monday following the game. Explanations not filed by this date will be subject to the consequences as outlined in the JSFC Constitution for Lopsided Score Rule Infractions.
Lopsided Score Rule Grievance - This form is to be completed by the head coach of any team that loses a game by 35 points or more if the coach and program would like to have the outcome of the game formally reviewed by the competition committee for bylaws infractions. This fully completed form must be emailed to the competition committee by Wednesday following the game or the Grievance will not be considered.
Roster Certification Documents
2010 JSFC Roster Upload - This document should be completed for each team you will be entering into the conference (JV and or Varsity). All fields should be completed for each player. In the notes section you should enter the school the player will be attending in the fall preceeded by a capital R if he is a returning player. No: should be filled with the players jersey number for the upcoming season. Team should be completed with the player's team alias as noted in the "JSFC Website Team and Playing Field Alias" form.
2010 Roster Upload Sample - Explains and gives examples of how the 2010 Roster Upload document should be completed for each team entered into the JSFC.
JSFC 2010 Age and Grade Requirements * - A quick refernce of the grade players must be in to qualify to play on the JV or Varsity. It also notes a birth date range quick reference to show eligibility to qualify to play on the JV or Varsity. Both requirements must be strictly adhered to.
JSFC Proof of Residency * - One Executed Copy to be sumbitted for each player appearing on the JV and Varsity Roster. Completed by Parent or Legal Guardian and then co signed by an authorized officer of the program the athlete is participating in. Used to verify residency.
JSFC School and Grade Verification Form - Can complete this docuent with each players name, grade and school and have an administrator from the school sign off on it to attest to the grade, school and legal residence of players appearing on the JV or Varsity Roster. This can be done in lieu of sumbittiing a copy of each player's previous year's report card.
Roster Certification Requirements * - This document explains the documents required to certify a player is eligible to be included on a program's Official JV or Varsity Roster for the upcoming season.
